We are in the centre of Cumbernauld close to the main shopping centre in a large Health Centre.

Coming from either Glasgow or from the east, follow the signs into the centre of Cumbernauld to find the Health Centre.

When you go into the main entrance of the Health Centre ignore the reception desk straight ahead and turn left until you come to another reception desk. This is signposted "Dr Graham and Partners". Report to the receptionist there.

What to remember when you come for your HGV medical or taxi medical...

If you wear glasses for driving bring them for the eyesight test. If we've sent you a sample bottle, remember to bring a urine sample with you. If not, we'll ask you to do a sample when you arrive, so don't go just beforehand.

Make sure you have your medical form for the doctor to fill in. Please fill in the section with your name and address and GP details before you arrive. (If you have a DVLA D4 medical form this is on the back page.)

Bring identification with you, such as a passport or photo driving licence. If you don't have either please call us.
